  • Family Literacy Activity Logs - Directions: This form is to be completed by the Child Tutor and the parent. Please write the literacy activity that you did with your child(ren) and the number of minutes you spent with your child(ren) involved in that activity each day. 
    • Activity Log in DOC form to print, fill out & submit manually
    • Activity Log in Web form to fill out & send online

Personal Education Plans & Goal Progress Reports

• Initial Personal Education Plans (completed at intake)
• Goal Progress Report (completed every 6 months)
